Bi Pride accessories/symbols - what are their origins/history, meanings etc?

Saw a thread earlier this week where some AE members posted some pics of "bi pride accessories".

It's the first time I'd seen these things and would like to know more about them -- the colours, their origins/history and so on.

• Why those colours specifically? 
• Are they always in that order?
• What are all the different bi pride symbols? 
• Who came up with it & when?
• Is there a specific meaning behind each colour?
• Are the accessories/symbols recognized globally or is it a North American thing?
